How the groups were formed
A prospective study of 80 adults undergoing flexible bronchoscopy compared ketofol, a ketamine–propofol mixture, with midazolam. The care team selected the regimen based on clinical factors that included anticipated airway tolerance, procedural requirements, comorbidities and clinician familiarity. The decision was recorded before the procedure, but allocation was not random.

Desaturation below 90% occurred in 5% of the ketofol group and 60% of the midazolam group. The investigators also reported fewer cough events and faster recovery in the ketofol group. The size of the observed difference does not remove the possibility that treatment selection or features of the procedure influenced the comparison.
What to compare in a disputed case
Locate the reason for the chosen regimen and compare baseline oxygen needs, airway findings, procedure timing and monitoring. Examine what the chart records at the first sign of a drop in oxygen saturation. The published rates describe the groups as treated at one center and cannot establish what would have happened under a different regimen in an individual case.
