What does an as-needed ICU sedation protocol tell us about a medication administration record?

An ICU may target the same sedation score using intermittent boluses or a continuous infusion. A protocol comparison makes clear why the label on the order should be separated from the drugs and amounts actually administered.

The comparison in one ICU

A retrospective quality-improvement evaluation compared an approach that prioritized as-needed boluses with initiation of continuous infusion. It evaluated 254 ventilated patients with a light-sedation target on the Riker Sedation–Agitation Scale. The researchers examined patient-level drug requirements and clinical outcomes, alongside unit-level pharmacy charges.

Which entries answer the case question

The sedation target describes the intended clinical state. The administration record identifies bolus doses and infusion start, adjustment and stop times. Recorded assessments describe observed depth. The single-center retrospective design gives evidence about implementation and outcomes under local practice, while the individual chart is needed to reconstruct actual exposure.
