Separate the first dose from the total

A retrospective study followed 196 adults treated for status epilepticus at one emergency department. Researchers distinguished the first benzodiazepine dose from the total administered. Only 34 patients, about 17%, received a first dose meeting the study’s criteria. Some patients had also received benzodiazepines before reaching the hospital.

The outcomes depended on which comparison was made

The study found no significant difference in intubation or mortality when groups were defined by first-dose adequacy. The authors reported a mortality association when they grouped patients by total dose adequacy. The initial and cumulative comparisons address different questions, and events during treatment can influence whether more medication is given.

Build the timeline from administration records

Reconcile emergency medical services and hospital records, and record the drug, route, weight used for assessment, amount and time of each dose. Place seizure observations, other antiseizure therapy and airway interventions on the same timeline. A retrospective association cannot establish whether a different dose would have changed the outcome of an individual case.
